Return to sport after cruciate ligament surgery

🏃 Return to Sport After Anterior Cruciate Ligament Surgery: How to Do It Right?

You’re about to have your anterior cruciate ligament operated on and you dream of returning to your favorite sports? This article explains how to get back to activity safely and progressively. Yes, it’s possible! But you need to respect the stages.

First, Understand the Anterior Cruciate Ligaments

The anterior cruciate ligaments are two X-shaped structures inside your knee. They play a crucial role:

• Stabilize your knee during pivots and direction changes

• Limit excessive forward movement of your tibia relative to your femur

• Allow you to practice sports without risk of knee dislocation

The anterior cruciate ligament (ACL) is the most frequently injured in sports (football, basketball, skiing, etc.). A rupture means your knee is no longer stable and surgery is necessary.

After Surgery: The Recovery Phases

0-3 months : Recovery

Gentle rehabilitation, recovery of mobility in flexion and extension. Strengthening exercises in unloaded position then with body weight. No contact sports at this level.

3-6 months : Strenghtening

Continued muscle strengthening that becomes progressively more intense. Proprioceptive work. Low-pivot sports are resumed gently (walking, cycling, swimming, etc.).

6-9 months : Preparation

Sport-specific training approved by the surgeon (without returning to competition).

9-12 months : Progressive return

Depending on your progress, return to competition can be done.

The Main Problem: Returning Too Soon

⚠️ Many patients return to sports too quickly without completing rehabilitation. The statistics are alarming:

• Around 15,000 people in France undergo ACL reconstruction every year

• Among young athletes under 25, over 20% experience a relapse (injuring the same ligament)

• Most relapses occur within the first 6 months after returning to sport

The Crucial Role of the Physiotherapist

After ACL surgery, your physiotherapist is your best ally to:

• Assess your recovery with objective tests

• Plan your rehabilitation progressively and scientifically

• Prepare your knee for the stress of sports

How to Return to Sports Properly: Essential Criteria

✔️ Your quadriceps and hamstring strength = 90% of the healthy leg (measured)

✔️ You have full range of motion

✔️ You pass jump and balance tests

✔️ You tolerate stops and pivots without pain

✔️ Your physiotherapist and surgeon agree

Key Points to Remember...

✔️ Your ACL needs 9-12 months minimum to heal properly

✔️ Returning too soon = 20% risk of re-injury before age 25

Progressivity: The Key to Success

WEEKS 1-2 : Return to 50%

• Training at 50% of normal intensity

• Simple movements without pivoting

WEEKS 3-4 : Increase to 75%

• Training at 75% intensity

• Introduction of simple pivots

WEEKS 5+ : Full Intensity

• Training at 100% without restriction

• Competition if all criteria are met
